For your convenience, we have an optional pre-printed label of the laboratory’s mailing address that you can simply download, print, and glue or tape to the front of your envelope for your hair sample submissions. You would need a black & white or colour printer to print this label using either an A4 or Letter size paper.

FAQ

What do I need to enclose with my hair sample?

You would need to print a copy of your submittal form and enclose it in your sealed main envelope before mailing your hair sample to the laboratory.

What if I didn't fill out a submittal form?

It is mandatory to enclose a completed submittal form with your hair sample submissions to the laboratory.

Your sample may be discarded and your order canceled without a refund if your hair sample is received without a completed submittal form.

What if I have misplaced my submittal form?

If you had previously filled out a submittal form after making a purchase of an HTMA Profile, you should have received a soft copy via the email address you used to register your details. Check your “Promotions” and “Spam” folders if you do not see it in your Inbox.
