HTMA Order Instructions

Please download the Hair Sample Collection Envelope here. You will need it to store and send your hair sample safely.

No worries if you do not have access to a printer. Place your hair sample in a clean envelope, seal it, then print (in block letters) your Name, Reference No. 6663, your Lab No., if this is a retest, on the front of the envelope.

Note that every HTMA sample submission must be accompanied by a printed hard copy of the submittal form. Ensure that your form was accurately filled out.

Next, you would need to print a hard copy of your completed submittal form which was emailed to you and enclose it in a envelope with your hair sample. Not doing so may result in a processing delay or a non-refundable cancellation of your order.
